Evidence for the CQC Safe key question
Safe asks whether people are protected from abuse and avoidable harm. CQC groups it into eight quality statements. The lists below suggest typical evidence; your own records should reflect how your service actually works.

The eight Safe quality statements
- Learning culture — incident reviews, lessons learned, near-miss reporting
- Safe systems, pathways and transitions — admission, discharge and hospital transfer records
- Safeguarding — referrals, tracker, training and staff knowledge
- Involving people to manage risks — person-centred risk assessments agreed with people
- Safe environments — health and safety, fire and equipment checks
- Safe and effective staffing — rotas, dependency, safer recruitment, training
- Infection prevention and control — IPC audits, cleaning records, outbreak plans
- Medicines optimisation — MAR audits, competency checks, medicines reviews
