Safeguarding tracker

A safeguarding tracker follows each concern from the moment it is raised to the moment it is closed. Many concerns involve several organisations and take weeks, so a tracker stops them drifting. It also lets you spot patterns — for example, repeated concerns about the same staff member or unit.

MyCareCompliance brings practical CQC audits, evidence, workforce oversight and improvement actions into one platform for adult social care providers.

7 days free. Plans from £24.99 per organisation per month, inclusive of VAT. Features vary by plan. Compare plans and trial terms.

Who it applies to

All adult social care providers. The local authority leads safeguarding enquiries under section 42 of the Care Act 2014; providers must refer, cooperate, and protect people in the meantime.

What to record for each concern

  • Date raised, who raised it and category of abuse
  • Immediate protective action
  • Date referred to the local authority and reference
  • Whether CQC was notified, and the date
  • Police involvement
  • Your internal investigation or contribution to the section 42 enquiry
  • Outcome and any actions agreed at safeguarding meetings
  • Date closed and lessons learned

Common findings

  • Concerns dealt with internally without referral
  • Referrals made but not notified to CQC
  • Actions agreed at strategy meetings not followed up
  • No overview of themes

Internal audit questions

  1. 1.Were all concerns referred to the local authority as required?
  2. 2.Was CQC notified of abuse or allegations of abuse?
  3. 3.Was immediate protective action recorded?
  4. 4.Are agreed actions complete?
  5. 5.Are themes reviewed at governance meetings?

Practical checklist

  • Open concerns reviewed weekly
  • References and dates complete
  • Notifications cross-checked
  • Actions tracked to completion
  • Themes reported monthly

How MyCareCompliance can help

In MyCareCompliance, safeguarding concerns are recorded in the incident register with a safeguarding referral flag, and follow-up work is tracked as improvement actions with 'Safeguarding' as the source. Both feed the dashboard and governance report.
