Risk assessment audit
A risk assessment audit checks that risks to people are identified, proportionate and kept up to date — and that staff actually follow the control measures. Regulation 12 expects providers to assess risks and do all that is reasonably practicable to reduce them, without restricting people more than necessary.
MyCareCompliance brings practical CQC audits, evidence, workforce oversight and improvement actions into one platform for adult social care providers.

Who it applies to
All adult social care services. The audit should cover individual risk assessments (falls, pressure areas, choking, moving and handling, self-neglect) and environmental or service risks (premises, lone working, fire).
What to look for
- A recognised tool used where one exists, for example for pressure area or nutrition risk
- A clear description of the risk, the controls and who does what
- Positive risk-taking recorded where the person chooses to take a risk, with their involvement
- Review after incidents: a fall should lead to a falls risk review
- Control measures reflected in the care plan and daily practice
- Environmental and fire risk assessments in date, including personal emergency evacuation plans
Common findings
- Risk scores recorded with no plan to reduce them
- Assessments not reviewed after incidents
- Blanket restrictions with no individual justification
- Staff unaware of the controls in place
Internal audit questions
- 1.Is every identified risk assessed with clear controls?
- 2.Were assessments reviewed after incidents or changes in health?
- 3.Are restrictions the least restrictive option and justified for the individual?
- 4.Do staff know the controls for the people they support?
- 5.Are PEEPs and environmental assessments current?
Practical checklist
- Individual assessments sampled
- Incident log cross-checked for reviews
- Environmental and fire assessments checked
- Restrictions reviewed
- Actions assigned and re-check date set
How MyCareCompliance can help
MyCareCompliance has a Risk Assessments Audit template, a service risk register with a likelihood-by-impact matrix, and risk assessment records. Incidents can be linked back to risk reviews so you can show the learning loop.
